Field BasedInstead of operating out of a clinic, our highly mobile team provides services directly on the streets where they are needed most. Reducing the barriers of access to health care while delivering ongoing customized and coordinated support to maximize stability long-term as they move into housing.
24/7 SupportRecognizing that crises do not always happen during regular business hours, our team has a 24/7 on-call number and staff to respond to emergencies. Coordinating incoming referrals from first responders and helping to ensure proper discharge planning from emergency room care.

Integrating with the community to achieve improved health, wellness, and quality of life. Meeting people where they are on the recovery spectrum, we embody the platinum rule: "Treat people the way that they would like to be treated."
CustomizationLos Angeles' homelessness crisis is a decades-old issue, and each person has unique needs. Our team bridges the gaps people often fall through by providing multidisciplinary services for as long as they need them. We help people meet their evolving goals by collaborating on recovery plans as they transition from the streets into housing.

All the Tools Our Communities Need to Succeed, a Fraction of the Cost
Our innovative approach has the power to minimize costs while maximizing outcomes. Implementing a Recovery Oriented Documentation Standard and value-based funding system rather than the existing "bill-for-services." Each Member served by the Double Secret Project receives unlimited support for roughly $60,000 annually or about $170 per day. Approximately 1/10th the cost of existing Medi-Cal funded Adult Full Service Partnership programs.
The Double Secret Project assists adults age 24 - 65+ who have been diagnosed with a mental illness and would benefit from intensive outpatient services in a community setting. We expand access to psychological and physical health care, substance use counseling, housing navigation and retention, legal aid, peer support, community integration, and supported employment. Our program helps navigate these complex systems to maximize successful outcomes for individuals experiencing chronic homelessness and frequent judicial system involvement.
We intend to launch as a 5-year pilot study to test our model of funding, documentation, and intensive integrated services against treatment as usual. During the foundation year, our team will enroll 100 Members before doubling membership in the second year to serve 200 Members and maintaining a capacity of 300 Members for the remainder of the study.
Our study will track outcome measures surveyed annually from Staff providing care, Members receiving services, and Community Stakeholders. By the end of year three, we anticipate seeing significantly improved recovery levels, quality of care, perception of services, and reduced numbers of arrests and hospitalizations compared to people receiving care within the existing system. By the end of year five, we hope to have developed enough proof of concept to continue expanding throughout Los Angeles County in the hopes of transforming mental health services within California and the United States.
